Steps:

1. Click the applicable button to download the form and return it to us by e-mail, fax, or mail. If you are self-represented, please fill out the self-represented form as well as the QDRO request form.

2. Your forms will be reviewed and we will get back to you with an estimate.

3. If you want to proceed you can pay through the website or send us a check. Work does not begin until you have paid in full and have provided all that we have requested.

NOTE REGARDING JOINT CASES: Pursuant to Family Code § 216, if Law Office of James K. Moore is jointly retained all communication with us needs to include the other side (i.e. both parties if self represented, or both attorneys). In other words, we cannot have ex-parte communications.

Law Office of James K. Moore is not responsible for your attorneys’ fees for QDRO preparation, court filing fees, or costs of QDRO pre-approval. Unless you have a written fee agreement retaining Law Office of James K. Moore to represent you as your attorney of record, you take your attorneys’ fees on yourself and our scope is limited to QDRO preparation.
